Please be aware that we need the NAME and EMAIL address for each Member. If your order is for one Membership (Standard or Premium)  we will assume the details provided at Checkout. If there is more than one Membership ordered, please write each person's NAME and EMAIL in the Instruction/Special Request box at the Checkout stage
 
Completing Multiple requirements:
